Application for financial aid from X-in Miao Charity Project
Targeted applicants
Patients who are applying must be unable to afford the treatments due to low-income.
X-in Miao Charity's team will determine whether a patient needs conservative scoliosis treatments or surgical treatment.
Declaration of applications
Aged over 18 applicants must fill out their applications by themselves. Patients under age 18 can have a guardian/parent to help fill the application. All application files must stay true and complete.
The charity will only cover part of the medical cost. Transportation costs and lodging fees must be covered by the patient.
All information filled in the application must stay genuine. If the information is found to be false or inaccurate, X-in Miao Charity retains the final decision to not grant any funds and pursue legal responsibility.
The management team of X-in Miao Charity is responsible for reviewing and filing all applications.
Applicants who receive granted funds must provide necessary background information, photos, videos, and etc.
The management team will explain the policies of "X-in Miao Charity Application Form Evaluation" to recipients.
This charity project is based on the application of the patients or their guardians, and their conditions. If a patient needs to engage in multiple conservative treatments, the charity can help cover one extra treatment once a year. Patients undergoing surgical treatment can also apply for funds that cover traction and surgical treatments.
Application condition
The charity's granted fund is opened to all ages and all nationalities.
Applicants must show their minimum living allowance permit by the government, or complete declaration form that shows the applicant's household income is insufficient to sustain daily needs and the form must be approved by at least two officials from the local village committee, county (district), township (street office), or Civil Affairs Department.
Aapplicants that have a disability permit, from a martyr's ward, sacrificed soldiers' family, family members of deceased soldiers, labor modeled family, orphan permits, or single-family certifications will be given first priority.
