Patient's ID or Residence Booklet (scan the front page and householder's page)

Identifications

1. Between minimum living allowance permit and declaration form that shows the applicant's household income approved by at least two officials from the local village committee, county (district), township (street office), or Civil Affairs Department, patients must choose either to show their household income status. 
2. Please provide If you have the disabled permit, or come from martyr's family, sacrificed soldiers' family, family members of deceased soldiers, labor modeled family, orphan permits, or single-family certifications.

Agreement form

1. Patients above the age of 18 must sign this form themselves. Patients under 18 can have their guardians to help them sign the form.
